Our Support Coordination service is designed to help you make the most of your NDIS plan. As a capacity-building support, we work alongside you to:
- Understand and navigate your NDIS plan to achieve your personal goals.
- Connect you with NDIS providers, as well as community, mainstream, and government services.
- Empower you with the skills and confidence to manage and coordinate your supports independently.
Coorination of supports
Our Support Coordination service is here to build your capacity in using and managing your plan. A dedicated Support Coordinator will work with you to ensure a tailored mix of supports that helps you strengthen relationships, manage service delivery, live more independently, and become actively involved in your community.
